What makes an 8D survive customer review

A practical checklist for root-cause logic, escape cause, evidence, and corrective actions that hold under scrutiny.

Customer-facing 8Ds fail less often on formatting than on logic. The problem statement, root cause, and corrective action must form a closed chain of evidence.

Separate occurrence cause from escape cause. A strong technical root cause still fails if detection and containment weaknesses are unexplained.

Ask: would a skeptical quality manager accept this without a plant tour? If not, the evidence layer is incomplete.

Corrective actions must change a control — process, design, detection, or supplier control — not only retrain and rework.
